This is the second commit in preparation for changing cdEXIT into a C++ exception.

source/cddefines.h:

Redefine cdEXIT to print exit message.

source/*.cpp:

Remove all calls to puts.

tsuite/auto/checkall.pl:

Two minor changes to improve detection of normal exit or failure.
